Affidavit of Identity

An Affidavit of Identity is used when you have lost all of your pieces of government issued photo identification. When you go to the government to get new documents, the government office will ask you to swear what is called an "Affidavit of Identity". 

Notary-toronto.ca can help! This process involves several steps to complete. You will need to prepare an Affidavit of Identity where you swear you are who you say you are. In addition, you will need to bring someone who knows who you are and who also has a piece of current government issued photo identification. At the same time as when you swear your oath, the other person will also swear that they know who you are and can attest to your identity. This other person will have to present their piece of government issued photo identification. It is best to bring a family member, however, a friend who has known you for some time can also be an effective party.
